We are a Patient Centered Medical Home (PCMH). This means our staff and the Primary Care Provider (doctor, nurse practitioner, or physician assistant) are on your team and will help you meet your healthcare needs. Think of us as your “home base” for care. We will do our best to make sure you get the attention you need to stay healthy and get you feeling better when you are sick. Please call us before seeking care elsewhere and we will see you in our office or work with the specialist you need to see. We will help you set goals to improve your health and support you in meeting those goals. If you need extra help we will assist you in finding it. We are a team and you are a key player.
